To request a business hazardous waste appointment:

Read through the BHWD Resource Packet to make sure you are eligible to use the Business Hazardous Waste Program. Call (970) 498-5773 with questions.

Download and fill out the “Waste Inventory Form.” (Need help filling out the form? Check out this example or call (970) 498-5773.)

If any of your waste is a mixture of different chemicals or the result of a process, complete a "Spent Waste ID Form" for each distinct waste and attach it below, along with Safety Data Sheets (SDS) for all chemical constituents.

A staff member will be in touch with you by email or phone within one week.

You are NOT confirmed for a business hazardous waste appointment until you receive confirmation from staff.
